The development of the network has allowed for a multi-disciplined team who work across our member practices enabling better delivery of care across the area.

The pharmacy team is both Clinical Pharmacists and Pharmacy Technicians. They work directly with you as part of the GP team to make sure your medicines help you to get better and stay well.

Clinical Pharmacist

Our Clinical Pharmacists are highly qualified experts in medicines and can help people in a range of ways. These can include working with Care Home staff to improve the safety of medicine prescribing and administration. Liaising with both hospital pharmacy and the community pharmacy team to improve patient care.  Carry out Structured Medication Reviews with patients with ongoing health problems including those on multiple medicines to ensure they are being prescribed the best medication for their condition in a person-centred consultation.

Pharmacy Technician

Our Pharmacy Technicians main role is to ensure prescribing processes and services improve the quality of patient care; they do this with guidance from our Clinical Pharmacist. Their work includes gathering and preparing information for structured medication reviews untaken by the pharmacists, ensuring medications are correctly recorded on the GP systems when a patient leaves hospital to ensure the patient receives a high standard and continuity of care. They audit prescribed medication in response to changes to guidelines or safety alerts to ensure patients are made aware quickly so that changes can be made if necessary, supporting practices with medication queries whilst making sure prescribing activities are efficient, safe and cost effective for the NHS.

Mental health picture .jpegCurrently the team comprises of a Mental Health Practitioner and a Mental Health Care Coordinator.

The Mental Health Practitioner works to ensure the right level assessment is undertaken for diagnosis, treatments, and the patient’s journey.  Alongside the practitioner, the Mental Health Co-ordinator works jointly with individuals with low mental health facilitating access to services, advice, and information, focused on the individual.

Both help people access services and help available to patients.

The Social Prescriber works individuals giving them time to focus on what matters to the person to make positive changes in their lives and within their communities by linking people to activities, voluntary and community groups and public services.

This can be support can involve individuals to feel more involved in their community, meet new people or make some changes to improve their health and wellbeing

They connect people to community groups and services for practical and emotional support.  They collaborate with local partners to support community groups to be accessible and sustainable and help people to start new groups.

Physio therapy .jpegOur First Contact Practitioner service is provided by a registered and experienced health professional in the practices.  They are the first point of contact for patients, with musculoskeletal (MSK) conditions in the GP practice. These tend to be conditions that affect the bones, joints, muscles and spine.

Having them as part of the primary care team enables patients to be seen by the right person, they receive appropriate care providing patients with faster access to the right care. 

FCPs will refer for courses of ongoing physiotherapy treatment if appropriate. Where necessary, further specialist assessment or investigations

The team has access to two paramedics and two care co-ordinators.  Their current responsibilities are to support residents and relatives of an allocated number of care homes based in central Lincoln which are:
