Lincoln Healthcare Partnership are proud of the successful delivery of the vaccine programme.
The PCN was one of the early sites to mobilise and commence delivery the vaccine for the PCN population in December 2020. This was in conjunction with the continued provision of primary care throughout COVID developing the team to support and enhance delivery of primary care. The network continues to actively collaborate with the wider healthcare system on a number of projects, including the Holistic Health for the Homeless Team. The team has been instrumental in delivering the COVID vaccination to this vulnerable group working in partnership with other agencies such as LPFT, Lincoln City Council, ICB and the Neighbourhood team.
